17β-ヒドロキシエキセメスタンは、エキセメスタンの主要な活性代謝物質です。これは、サイトクロムP450(CYP)アイソフォームCYP1AおよびCYP4A11によるエキセメスタンの代謝によって形成されます。 17β-ヒドロキシエキセメスタンは、人間胎盤微小体を用いた場合の芳香族化酵素阻害剤(IC50 = 69 nM)であり、アンドロゲン受容体(AR)アゴニスト(IC50 = 39.6 nM)でもありますが、ERαに対して選択的であることが示されています。(ERα; IC50 = 21.2 μM)。 AR陽性およびERα陽性のMCF-7細胞(EC50 = 2.7μM)、T47D乳癌細胞(AR介在増殖:EC50s=0.43nM; ER介在増殖:1500nM) の成長を刺激し、テストステロン処理下で芳香族化酵素過剰発現する MCF-7aro 細胞の増殖を濃度依存的に抑制します。また、17β-ヒドロキシエキセメスタン(20 mg/kg)は、去勢ラットにおいて血清コレステロールおよびLDL値の上昇を抑制し、腰椎骨密度や大腿骨、第5腰椎の屈曲強度と圧縮強度の低下を防止します。
