Vaborbactam (RPX7009) ist ein Breitband-β-Lactamase-Hemmer mit besonders starker Wirkung gegen die Enzyme KPC, CTX-M, SHV und CMY[1]. Vaborbactam ist ein kompetitiver Inhibitor der zyklischen Boronsäure-Pharmakophore β-Lactamase und wird zur Behandlung komplizierter Harnwegsinfektionen verwendet[2]. Vaborbactam bildet eine reversible Koordinationsbindung mit β-Lactamasen und wird von β-Lactamasen nicht hydrolysiert[3]. Vaborbactam kann die Aktivität von -Lactamase gegen Mykobakterium abscessus verstärken[4]. Vaborbactam wird oft in Kombination mit Meropenem angewendet und zeigt in vitro eine hohe Wirksamkeit gegen resistente gramnegative Erreger, besonders gegen Klebsiella pneumoniae, das Klebsiella pneumoniae-Carbapenemase(KPC) produziert. Die MIC50-Werte liegen typischerweise bei ≤ 0.06µg/ml[5].
In vivo wurde Vaborbactam (6.25-100mg/kg) Mäusen intraperitoneal injiziert, um ein 24-stündiges Oberschenkelinfektionsmodell mit Klebsiella pneumoniae KP1094 zu behandeln. Die Kombination mit zwei Meropenem-Therapien(100 und 300mg/kg) führte zu einer dosisabhängigen bakteriziden Wirkung mit einer maximalen bakteriziden Wirkung von 2.50 log KBE(Koloniebildende Einheit)/Oberschenkel[6].

Vorbereitungsmethode | Ein 24-Stunden-Oberschenkelinfektionsmodell wurde mit Klebsiella pneumoniae KP1094 etabliert.
