PPACK es un potente inhibidor selectivo e irreversible de la trombina que inhibe la alfa trombina humana con un valor de Ki de 0,24nM.PPACK inhibe la unión de rt-PA a inhibidores de proteasa plasmática fuera del cuerpo[1-3].Forma un enlace covalente al sitio activo de trombina Ser y se cruza con His57 en el sitio activo para formar un complejo tetraédrico PPack-trombina.PPACK inhibió completamente los cambios en los productos de degradación de fibrina,plasminógeno y alfa 2-antiplasmina[4-6].
La coincubación con PPACK (0,2mM;2h)redujo el aumento de la permeabilidad endotelial inducida por la plasmina en un 59% en las células endoteliales de la aorta bovina[7]. La hemo oxigenasa (HO)-1 es una enzima limitante de la velocidad inducible por estrés en la degradación del hemo que confiere citoprotección contra la lesión oxidativa.El pretratamiento de las células con PPACK(24h; 30 nM) antagonizó efectivamente el efecto potenciador de la trombina sobre la expresión de HO-1[8].
PPACK(dosis en bolo de 10-100 μg/kg o infusión de 0.25-1.0 μg/kg/min) puede inhibir el tromboembolismo cerebral inducido por trombina en los conejos[9].
