Ciraparantag (PER977) est un agent de la réversion anticoagulante de large spectre, de type molécule synthétique de faible poids moléculaire et hydrosoluble. Ciraparantag neutralise et se lie à l'activité anticoagulante de l'héparine non-fractionnée, de l'héparine de bas poids moléculaire, du fondaparinux, ainsi que des inhibiteurs directs du facteur Xa et des inhibiteurs directs de la thrombine, grâce aux liaisons hydrogène non-covalentes et interactions charge-charge, de cette manière, restaurer la fonction hémostatique sans exercer d'effets procoagulants significatifs.[1-2]. Ciraparantag est indiqué pour la prise en charge de la réversion anticoagulante chez les patients présentant une hémorragie sévère ou devant subir une intervention chirurgicale urgente en raison d'un traitement anticoagulant[3-4].
In vivo, dans un modèle d'hémorragie par transaction de la queue chez les rats SD, une injection intraveineuse unique de Ciraparantag (1.25–31.25mg/kg) a été administrée soit au moment de la concentration maximale (Tmax) de l'anticoagulant (avant la lésion), soit après la lésion. Les anticoagulants utilisés comprenaient de l'édoxaban oral (10mg/kg), du dabigatran oral (37.5mg/kg), du rivaroxaban oral (5mg/kg), de l'héparine non-fractionnée intraveineuse (1–2.5mg/kg), ou de l'exoxaparine intraveineuse (10mg/kg). Ciraparantag a significativement réduit les pertes sanguines chez les rats[5].
