El NAD+ es una coenzima que transfiere iones de hidrógeno y participa en muchas reacciones enzimáticas. Está compuesto por ribosil nicotinamida 5'-difosfato acoplado a 5'-fosfato de adenosina a través de un enlace de pirofosfato[1]. El NAD+ es la forma oxidada de la nicotinamida adenina dinucleótido (NAD), y la forma reducida de NAD es NADH. El nivel intracelular de NAD+/NADH refleja la actividad metabólica y la salud de la célula[2]. El NAD+ es un transportador clave de reacciones redox, desempeñando un papel insustituible en la glucólisis, gluconeogénesis, el ciclo del ácido tricarboxílico y la cadena respiratoria[3]. El NAD+ también es un sustrato de la enzima nuclear poli (ADP-ribosa) polimerasa-1 (PARP-1)[4].
In vitro, el tratamiento de células T CD4+ naïve con NAD+ (100 µM) durante 72 horas indujo a las células T CD4+ naïve a producir una gran cantidad de citoquinas, promovió la diferenciación celular hacia células T auxiliares (Th1, Th2 y Th17), pero impidió que las células ya diferenciadas produjeran citoquinas[5].
In vivo, la administración intravenosa de NAD+ (10, 60 mg/kg) a ratones ART2−/− y de tipo salvaje (WT) redujo el efecto inhibidor de las células T reguladoras CD4+ CD25+ FoxP3+ (células T reg) en ratones WT de manera dependiente de la dosis, pero no tuvo efecto sobre la función de las células T reg en los ratones ART2−/−. También reguló el tamaño y el fenotipo del grupo de células T reg in vivo[6]. La administración intravenosa de NAD+ (10-20 mg/kg) en ratas sometidas a isquemia/reperfusión (I/R) redujo de manera dependiente de la dosis el infarto de miocardio inducido por I/R, disminuyó los niveles de señalización TUNNEL, Box y caspasa-3 clivada, y aumentó los niveles de Bcl-XL[7].
